Detail. This only happens if you download the client version before the server gets updated.

Try rebooting and running the game again. If the problem persists, then there’s nothing you can do.

There’s some random variation, but it’s not tied into any mission parameter.

I don’t remember but it was a Meteor Storm stage. Does UCOs appear from 75% difficulty at Double Teams only? Because I saw them lots of times below 75%, like at League and I met UCO 2 twice at the end of stage 3 and it was CI stage type too(at Weekly Challenge).

No, everywhere. I think there may be some confusion about which one “UCO 4” is:

“U.C.O.* encounter!” from 0%
“Another U.C.O. sighting!” from 25%
“Close Encounters” from 50%
“U.C.O. approaching!” from 75%
